Description

Amaryl is used for the treatment of type 2 diabetes mellitus (non-insulin-dependent diabetes) to improve the level of blood sugar when a suitable control cannot be reached with a diet and exercise alone. The active ingredient present in the formulation is Glimepiride, an insulin secretagogue and the representative of a sulfonylurea drug class. The medicine helps lower the level of sugar in the blood by increasing the amount of insulin produced by the pancreas and stimulating a better response of peripheral tissues to insulin.

Amaryl also has an extrapancreatic effect on glucose metabolism that exerts in improved peripheral glucose uptake and a decreased endogenous glucose production. To optimize glycemic control, the drug can be administered in a combination with insulin or other oral diabetes medicines containing Metformin. Amaryl may also be used in an off-label way to treat pregnant women with gestational diabetes mellitus.

Indications

Treatment of type 2 diabetes mellitus in adult patients where blood sugar levels cannot be properly sustained along with a diet, physical exercise, and weight reduction alone.

The medication is also used off-label in the treatment of gestational diabetes mellitus in women, who do not have a satisfactory glycemic control despite following a proper diet and physical activities.

MOA (mechanism of action)

Glimepiride is a medium-to-long-acting, ‘second-generation’ sulfonylurea agent with a hypoglycemic effect. Its mode of action in lowering blood sugar is performed by stimulating the release of insulin from the pancreatic beta cells, and increasing sensitivity of peripheral tissues to insulin. Glimepiride binds to ATP-sensitive potassium channel receptors, located on the pancreatic cell surface, which leads to a closure of the channels. This depolarizes the membrane and triggers calcium ion influx through voltage-sensitive calcium channels which results in an increased intracellular calcium ion concentration followed by an exocytotic release of insulin. Glimepiride also exerts extrapancreatic activity with its ability to decrease hepatic glucose secretion and glycosylated hemoglobin levels in the bloodstream.

Dosage

The course of treatment with Amaryl should be initiated for each patient individually according to the results of blood and urine sugar tests. The medication is to be taken before breakfast, or if none is taken, the first main meal of the day. The tablets must be swallowed, without chewing, with a glass of water.

Maximum dose must not exceed 6 mg a day.

The medication should be used following a proper diet and exercise regimen without skipping meals. Diabetic patients’ condition should be monitored by a healthcare specialist and their blood and urine glucose levels must be checked regularly to maintain the optimal dosage of the medicine. The drug is meant for a long-term administration and should not be discontinued without a doctor’s recommendation so to avoid a rapid increase in blood sugar.

Missed dose

Store at room temperature under 30°C (86°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Contraindications

Do not take Amaryl if you are hypersensitive to Glimepiride, any other product’s ingredients or other sulfonylurea drugs (e.g., glibenclamide).

Do not use the medication in type 1 diabetes mellitus, diabetic ketoacidosis (a serious diabetes mellitus complication when cells in the body are unable to get the glucose they need for energy as there is not enough insulin).

Do not take the medicine if you are in a diabetic pre-coma or coma (a life-threatening diabetes complication that causes unconsciousness).

Do not take the tablets if you suffer from serious renal, hepatic, thyroid, or other endocrine disorders.

Use the medicine with caution in case of severe cardiovascular diseases.

Do not take Amaryl tablets if you have a glucose 6-phosphate dehydrogenase deficiency (G6PDD) (a genetic disorder characterized by red blood cells break down when the body is exposed to certain drugs), as it may raise the risk of hemolytic anemia.

To avoid the risk of hypoglycemia, try not to take the tablets concomitantly with some drugs to treat pain and inflammation (azapropazone, aspirin-like medicines), medication for the treatment of urinary infections (like some long-acting sulfonamides), medicines to treat bacterial and fungal infections (such as tetracyclines, fluconazole, etc.), coumarin derivatives (like warfarin), anabolics, medicines used for male sex hormone replacement therapy or to treat depression (fluoxetine, MAO-inhibitors), lowering high cholesterol levels (fibrates) or high blood pressure (ACE inhibitors) medications, anti-arrhythmic agents to control abnormal heartbeat (disopyramide), to treat gout (probenecid, sulfinpyrazone) or cancer (ifosfamide), drugs for weight loss (fenfluramine), medicines to increase circulation (pentoxifylline), to treat nasal allergies (tritoqualine), sympatholytics for the treatment of high blood pressure, heart failure, or prostate symptoms.

In order to diminish the possibility of hyperglycemia occurrence, avoid the co-administration of Amaryl® with medicines containing female sex hormones (oestrogens, progestogens), thiazide diuretics (water tablets) for the treatment of high blood pressure and edema, drugs used to stimulate the thyroid gland (as levothyroxine), glucocorticoids for allergies and inflammation, medications to treat severe mental disorders (chlorpromazine), sympathomimetics, barbiturates, laxatives used against constipation, medicines to treat fits (phenytoin), medications for the treatment of increased eye pressure (acetazolamide), high blood pressure or low blood sugar (diazoxide), drugs used to treat tuberculosis (rifampicin) and severe low blood sugar levels (glucagon).

Be careful when taking Colesevelam, a medicine used to reduce cholesterol because it can decrease the absorption of Glimepiride. It is recommended to take Amaryl tablets at least 4 hours before Colesevelam administration.

Alcohol consumption should be limited as it may increase the risk of low blood sugar and cause undesirable side effects.

Avoid driving or operating machinery since your ability to concentrate and react may be impaired by hypoglycemia or hyperglycemia or as a result of visual deterioration.

Do not use the medication if you are pregnant or breastfeeding without consulting your doctor.

Do not take Amaryl if you are under 18 years of age.
